To get started with Shaping Change, LLC., contact us and let us know which service(s) you are interested in. Fill out our intake form and submit it to us with all of the documents listed on the intake form.

A member of our Shaping Change, LLC. office staff will contact you to schedule a complimentary consultation, where you will meet with a member of our clinical team and discuss your needs.

Our team will provide you with information about our services and the services that may be the best fit for you and your family. This is preliminary, so our recommendations may change as we learn more through our assessment and evaluation process.

After the consultation, we will schedule the assessment/evaluation. The treatment plan will then be developed and reviewed. From there, sessions can be scheduled.

The necessary evaluations and/or documentation needed to get started may vary by individual and insurance company, if you are going through insurance.

We try to request as much documentation during our intake process to learn more about each patient and to move the insurance authorization process along quicker.

We accept and are in-network with most major insurances.

Each plan is different and your individual benefits, eligibility, and coverage may vary.

Our office staff will help you navigate this process and work with you to make it as smooth and simple as possible.

We currently do not take any forms of Medicaid or Medicare.

The insurance process differs for each carrier.

Our Shaping Change, LLC. office staff is extremely knowledgeable about working with insurances and will collaborate with each patient throughout the process. For services to be covered by your insurance, authorization will likely be required.

This process can take up to 30 to 45 days when initiating services.
